Candidates for teams must present written, signed proof of a current physical exam in order to take part in the athletic program at the High School during the 2010/11 School Year. This means that the exam must be dated after August 1, 2009 for fall sports, after November 1, 2009 for winter sports and after March 1, 2010 for spring sports. An exam dated after March 1, 2010 will cover a student/athlete for all three (3) seasons during the 2010/2011 School Year. It is permissible for your Doctor to use his or her own physical form. Results of the physical exam should be mailed to the Athletic Office, either by a candidate's physician or
parent, as soon as possible upon completion of exam. The copy you send will be given to the Wellesley High School Nurse in September so there is no need to send an additional copy to the Nurse. You may also FAX the physical directly to the Athletic Department at: 781-446-6289.
2. A “Gold Form” which is our Conditions Governing Participation in Interscholastic Athletic Activities for the Wellesley Public Schools must be completed. These forms will be available in the Main Office at the Middle School and High School this summer. We WILL NOT accept the Gold Forms via FAX or EMAIL.
3. Be declared academically eligible by the Athletic Director. Report Cards from the previous term are used to determine eligibility.
Registration for all programs will be held in the Athletic Office at Wellesley High School from Monday, August 16, 2010 through Wednesday, August 18, 2010. Office hours: 8 am to 12 noon. (If registration forms have already been sent in to us, there is not need to come into the office.) Students must have completed steps one (1) and two (2) mentioned above in order to register. Registrations may also be mailed to us at: WHS Athletic Dept., 50 Rice Street, Wellesley 02481-6099.
Any student entering Wellesley High School for the first time this fall, with the exception of students coming into the 9th Grade from the Wellesley Middle School, should contact the Athletic Department during the week of August 16th so that athletic eligibility can be determined.
If you have any questions regarding physical exams, eligibility or any other aspect of the athletic program, please contact us.
Our direct office line is: 781-446-6305 ext. 5195. Our FAX number is: 781-446-6289.
