Spring Sport Registration
In order to try out, a student must have the following three (3) steps in place:
1. The State Interscholastic Athletic Association requires a Doctor's Physical Examination dated within thirteen (13) months of any practice or play. Candidates for teams must present written, signed proof of a current physical exam in order to take part in the athletic program at the High School during the 11/12 School Year. This means that the exam must be dated after July 23, 2011 for fall sports. An exam dated after February 18, 2012 will cover a student/athlete for all three (3) seasons during the 2011/2012 School Year. It is permissible for your Doctor to use his or her own physical form. Results of the physical exam should be mailed to the Athletic Office, either by a candidate's physician or parent, as soon as possible upon completion of exam. The copy you send will be
given to the Wellesley High School Nurse in September so there is no need to send an additional copy to the Nurse. You may also FAX the physical directly to the Athletic Department at: 781-446-6289. The physical now expires at 13 months instead of being good for the season. In the past if it was good at the start of the season it was good for the whole season. Please be sure to have an appointment set up so no time is lost.
2. A Gold Form which is our Conditions Governing Participation in Interscholastic Athletic Activities for the Wellesley Public Schools must be completed. These forms will be available in the Main Office at the Middle School and High School this summer. We WILL NOT accept the Gold Forms via FAX or EMAIL.
3. Be declared academically eligible by the Athletic Director. Report Cards from the previous term are used to determine eligibility.
If you have any questions regarding physical exams, eligibility or any other aspect of the athletic program, please contact us.
Our direct office line is: 781-446-6305 ext. 4614. Our FAX number is: 781-446-6289

For games and practices at Sprague Field - everyone should park in the Calvin Rd. Lot behind the Middle School and walk down the hill or in the actual Sprague parking lot after 3:30. There should be no cars parked in the turnaround area in front of the fields or in front of the maintenance garage with the exception of officials, coaches, athletic trainer and emergency vehicles. Thank you
